AST Support Worker - Hull




This role is an exciting opportunity to join an expanding team Housing support service in Hull




(The post will be based in Hull )




Job Profile




Salary: Starting salary £25,115, rising to £26,425 after 12 months and rising to £27,355 after a further 12 months




Main Purpose




To provide high quality housing support services to tenants to provide tenancy support to enable move on to more independence accommodation.


 




Specific tasks


 



  • Assist tenants when moving in, or out of Target accommodation.


 



  • Assess the circumstances, aspirations and needs of each tenant to create a personalised support plan to tailor support to their individual circumstances.


 



  • Act as keyworker offering advice and support to tenants to enable them to develop their skills for independent living.


 



  • Assist residents to maximise their income and to manage their personal budgets. Including support to claim welfare benefits.


 



  • Support residents to understand their rights and responsibilities under their occupancy agreements.


 



  • Support and motive tenants to encourage them to achieve personal goals and to promote independence.


 



  • Carry out regular tenant risk assessment / safety plans, document and record these on relevant systems and work to mitigate and reduce risks over the period of support.


 



  • Support planning, review these periodically to evidence progress towards agreed objectives.


 



  • Encourage and emphasize the importance of paying for household bills including Target Housing Rent (as required by the occupancy agreement). 


 



  • Support to gain skills to maintain their Target Housing property and monitor the consumption of their utilities incl. Gas and Electric.


 



  • Monitor compliance of tenants occupancy agreements. Raise issues of non compliance with tenant and management.


 



  • Monitor occupancy to ensure that properties have not been abandoned or damaged and take prompt action when it happens to avoid unnecessary costs to TH.


  • Liaise with Partner Agencies to work collaboratively to deliver focused, joined up support for those the people we support.


  • Manage, monitor and proactively address neighbour disputes.  Support and empower tenants to be part of their community.





  • Assist in the delivery and development of tenant activities.


 



  • Work to empower tenants, promote and foster professional working relationships with tenants, colleagues and external organisations.

  • Ensure all records are up to date, factual and contractually compliant.


 



    General tasks


     



    • Attend and participate in training sessions or activities as required.


     



    • Participate in team meetings and supervision


     



    • Perform other such duties as may be required to further the aims of Target Housing within the Health and Safety requirements.


     



    • Adhere to all of Target Housing’s policies and procedures in particular our Equal Opportunities, Confidentiality and Health and Safety policies.


     



    • Carry out other such duties as directed by senior managers as appropriate to the aims of Target Housing.
