The Role
You will be based in Hull and will deliver high quality support to individuals who have a learning disability and /or autism. The support will be provided in our accommodation, as well as in the community and other appropriate settings. You will have an allocated caseload and be responsible for completing risk assessments, support plans and other paperwork. You will play an instrumental part in empowering individuals to live more independently, co-ordinating support with other key agencies, attending multi-disciplinary meetings.
The support will include but not be limited to, assisting with daily living skills including shopping, attending appointments, accessing community resources, health care and accessing and dealing with benefits to maintain a safe and comfortable living environment.
