What you’ll do
As a Care Assistant specialising in hospital discharges, you will be the compassionate face of our care team, helping clients safely transition from hospital to home. You will provide essential support according to individualised care plans, assisting with personal care needs such as bathing, dressing, managing incontinence, and supporting with medication. Your role will also include practical tasks like preparing meals, helping with housework, and ensuring a safe living environment post-discharge.
What you’ll need
You don’t need previous social care experience for this role; we’re looking for compassionate individuals who have a desire to care. You must be adaptable, willing to learn, and able to work as part of a close-knit team. Your empathy and dedication are what matter most.
