OneCare is a professional Home Care provider that has built a reputation on quality. We have service users throughout London that require care and support to help them remain living in the comfort of their own home. We are now recruiting for a Care Coordinator to join their professional management team; they are looking to only employ the very best who have passion and truly want to make a positive difference to the life of older people.
It is essential that you have relevant experience and the knowledge of the Home care sector and have a good command of restoring software packages (knowledge of People Planner would be advantageous).

You will be the first point of contact for staff, service users and commissioning organisations. You will need to have the skills to resolve and manage issues as they arise and be able to prioritise your workload, be flexible for the needs of the business and work within a team setting and able to consider business constraints and service user needs. You will also need to have a consistent and compassionate approach with our service users at all times. Experience using restoring software preferably People Planner is highly desired.
Principal responsibilities
* Preparing and sending rotas to service users and care workers
* Ensure all calls are covered and making amendments as required
* Work with the care manager to pre-empt recruitment needs
* Monitoring staff and client communications
* Monitoring and managing the Rotas
* Responding to complaints and queries from service users
* Emergency assignment of workers to cover care calls when required
* Accepting, allocating and processing referrals for new work efficiently
* Interviewing potential Care and Support Workers and ensuring all applicant paperwork is fully completed as per statutory requirements
* Checking of all ID documents against application paperwork
* Liaise with carers/service users ensuring they are informed of any changes
* To participate in the growth and development of the business, achieving targets and delivery within budgets.
* Accepting, allocating and processing referrals for new work and liaising with social workers and other stakeholders.
* Have the ability to work in a multi-agency approach
* Completing care and risk assessment as required and processing changes and amendments to existing care plans
* Assigning care workers to service users appropriately.
* Working with other members of the team to ensure high quality service provision.
* Talking to the service users on a regular basis about their care
* Ensuring all services are compliant with legislation's, CQC standards and regulations and Keeping computer and paper files up to date and Maintain all office policies, procedures and systems in line with company guidelines.

* Excellent telephone manner and experience of using Restoring Computer Packages Preferably People Planner

* IT Literate and have attention to detail
Qualifications
You will possess a minimum of QCF Level 3 or equivalent or previous experience in a similar role. The experience of using restoring software is a requirement, preferably People Planner
